How do I purchase a copy of an Accident Report online?

Copies of accident reports can be purchased at http://www.docview.us.com until May 9th.  After May 9th, accident reports can be purchased at https://payments.clemis.org/extservices.

Get a Copy of a Report by Mail

Mail your request with the:

• report number
• name of person involved
• date and location of the incident
• $5 money order, certified check or business check
• self-addressed, business-size envelope to:

Fax Requests

We are unable to process requests for incident or accident reports by fax.  All reports must be obtained in person or by mail.  This does not apply to government agencies.
